    You must supply valid photo ID and a verified USI number. 

    If you did not undertake your original White Card training with Asset College/Asset Training Australia then you will need to provide your statement of attainment (for course CPCCWHS1001 or CPCCOHS1001A) issued by the RTO you completed your original training with.

    The fee for a replacement White Card is $50. We will send you an invoice on receipt of this completed form.

    ​Replacements can take up to 5 business days to be finalised, they are then posted to the address provided or are available for collection from our North Lakes QLD address only. Please indicate in the details box if you wish to collect the White Card when it is finalised.

  • Evidence of identity (EOI) is required before a replacement white card will be issued. The identification documents that we can accept MUST meet the criteria outlined in this Evidence of Identity Requirements Document from Workplace Health and Safety Queensland.

    Record Retention Requirements by Asset College
    We must keep records of all aspects of GCIT for 5 years. This includes but is not limited to:

    • the serial number of each GCIT card issued
    • the name, date of birth, residential address and contact details of each person that has been issued a GCIT card
    • the date of issue of each GCIT card
    • any evidence supporting the issue of a replacement GCIT card
    • all student training and assessment materials
    • evidence of resources used in GCIT assessment activities
    • student EOI
    • decisions by us to refuse to issue or replace a GCIT card.


    If requested, Asset College must provide the Regulator with any GCIT record as outlined above.
    These conditions apply to every organisation that issues GCIT White Cards in QLD.

    General Construction Industry Training (GCIT) means the GCIT course delivered face-to-face either in a classroom or by connected real time delivery, for the purpose of issuing a GCIT card.
